Job Details

A paraprofessional who provides non-medical personal care and assists with activities of daily living, and related services in the home for the sick and the disabled. The individual requires specific training, competence and performs assigned functions of personal support services and skilled care to the patient in a residence under the direction, instruction, and supervision of a registered nurse (RN).


Reports To:
• Reports directly to the Director of Clinical Services/Nursing Supervisor/RN.

Duties/Responsibilities:

  • Provides personal care support services authorized in the Plan of Care and permitted under state laws.

  • Assists the member with non-medical activities of daily living (ADLs), including bathing, dressing, grooming, oral hygiene, skin care, shaving, toileting, and feeding.

  • Prepares meals and snacks consistent with the member’s nutritional plan and assists with feeding as assigned.

  • Assists with ambulation, transfers, positioning, and use of adaptive or transfer equipment as trained and authorized.

  • Performs routine exercises and active range-of-motion activities as assigned and delegated by nursing staff.

  • Assists with medications that are ordinarily self-administered, including reminders, opening pre-set containers, and assistance with handling or ingesting medications, in accordance with policy and training (does not independently administer medications).

  • Provides supervision and companion services to ensure the member is not left alone when it is unsafe to do so.

  • Assists with instrumental activities of daily living (IADLs) and homemaking tasks essential to the member’s health and safety, including light housekeeping, laundry, meal cleanup, grocery shopping, and errands directly related to the member’s needs.

  • Maintains a clean, safe, and sanitary environment within the member’s personal living space.

  • Observes and monitors the member’s physical, mental, and emotional condition and reports change promptly to the RN or agency designee.

  • Recognizes and reports emergencies, injuries, abuse, neglect, sentinel events, variances, accidents, or unusual occurrences according to agency policy.

  • Accurately reads, understands, and follows the Plan of Care, Assignment/Service Plan, and agency policies and procedures.

  • Completes required documentation for each shift/visit accurately, timely, and legibly; obtains required signatures and submits documentation within established deadlines.

  • Maintains confidentiality of the member and family information in compliance with HIPAA and state program requirements.

  • Adheres to infection prevention and control practices, including proper hand hygiene and use of precautions as directed.

  • Participates in required training, in-services, staff meetings, case conferences, and quality management or QAPI activities as assigned.

  • Maintains professional conduct, appropriate boundaries, and respectful communication always, honoring member dignity, choice, and independent living philosophy.

  • Performs only those duties for which the caregiver has received appropriate training, authorization, and supervision.

  • Performs other related duties as assigned within the scope of the Plan of Care and Family Caregiver policy.


Required Skills/Abilities/Knowledge:

  • Empathetic, compassionate attitude toward the care of medically fragile or chronically ill individuals.

  • Ability to read, write, and follow verbal and written instructions and care plans.

  • Ability to communicate effectively with members, family members, nurses, and agency staff.

  • Ability to observe, recognize, and report changes in condition or safety concerns accurately and timely.

  • Knowledge of basic personal care techniques, hygiene practices, body mechanics, and infection control.

  • Understanding of confidentiality requirements and professional boundaries.

  • Ability to complete accurate and timely documentation.

  • Mature judgment and sound decision-making skills within the scope of role and training.

  • Flexibility and adaptability to meet the ongoing needs of the member and household environment

Education/Experience/Licenses/Certifications/ Requirements:

  • Must be at least 18 years of age or meet minimum age requirements per state regulation.

  • Maintains a current, valid CPR and First Aid certification.

  • Completes required training hours prior to providing care and ongoing training within required timeframes.

  • Maintains all required training documentation and certifications on file with the agency.
