Job Details

The Director of Patient Care Services (DPCS) is a Clinical Manager (CM) who assists the Administrator and is responsible for the supervision/oversight of all patient care services and personnel in an agency. The DPCS strives to provide the highest quality of patient care and oversight while achieving excellent outcomes in a patient-centered environment. In this role, you will report to the Area Director of Clinical Operations (ADCO).


Duties/Responsibilities:

  • Responsible for overall day to day clinical and management of the Home Health Agency ensuring supervision, competency, evaluation and planning for the agency clinical and staff

  • Ensures oversight clinically of: making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized care plan(s)

  • Understands and adheres to all applicable regulations, standards, and federal, state, and local laws and established Agency policies and procedures. Following accreditation standards as applicable

  • Develops personnel qualifications and assignment of personnel

  • Available at all times during operating hours and participates in all activities relevant to the professional services furnished, including the development of qualifications and the assignment of personnel

  • Able to supervise, guide and develop patient care, personnel requirements, competency skills

  • Understands the financial and business side of operations in order to cover for the Administrator

  • Consults with the Administrator in determining a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of patient care personnel while promoting a viable financial environment

  • Responsible for recruiting, hiring, evaluating and terminating personnel. Assists in instituting, interpreting and documenting disciplinary action

  • Responsible for the orientation of new patient care personnel

  • Assists in the planning and consultative needs of staff

  •  Ensures the implementation of new/changed policies and procedures and supports policies/ procedures and uses appropriate channels for changes of such policies

  • Responsible for the coordination of interdisciplinary team services

  • Understands/ensures that all Quality Assurance Performance Improvement (QAPI) meetings and reports are collated and presented

  • Ensures that all case conferencing efforts for patients are coordinated on a weekly basis addressing high volume, high risk and problem prone cases along with care risk type patients

  • Meets corporate goals for metrics on 5 Star, VBP, scorecards and financial metric goals as established with the administrator and the ADCO

  • Ensures completion of standardized testing, accurate documentation and staff knowledge/completion of best practices

  • Ensures that coordination of care and documentation occurs between staff and patient/family/representative and facility and other outside agencies

  • Ensures that the following information/instruction is given in writing to the patient/caregiver/representative: visit schedule with frequency of visits, complete Medication Profile, any treatments to be administered by agency staff (POC), any other pertinent instruction related to the patient’s care needs, and name and contact information of the agency’s Clinical Manager

  • Responsible for the coordination of Agency’s services with the services of other community agencies

  • Participates in studies and research and other administrative functions as assigned

  • Observes confidentiality and safeguards in all patient related information

  • Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved

  • Serves as a resource person to clinical staff

  • Develops a cooperative relationship and communicates effectively and professionally with physicians and referral sources

  • Investigates, resolves and reports any problem relating to patient care and/or employee well- being and reports to ADCO

  • Immediately investigates and resolves any accident, incident, lost articles, property loss, drug diversion, abuse/neglect and/or unusual occurrence(s) and reports to the Administrator and Compliance Department as applicable

  • Attends pertinent continuing education programs as assigned by Administrator or ADCO and shares information with staff

  • Monitors equipment for appropriate use and takes steps to keep misuse to a minimum

  • Responsible for the Agency’s ongoing QAPI program. Assists with the overall development and implementation of the QAPI plan, and the identification of goals related to patient outcomes and coordinating, participating, and reporting of activities and outcomes results

  • Functions as Agency infection control and EMS manager

  • Responsible for the development, implementation and evaluation of patient plans of care

  • Responsible for the review of patient medical records for compliance with federal, state and local laws, accrediting organization standards (if applicable), and Agency policies and guidelines

  • Assists Clinical Supervisor in conducting patient care conferences, staff meetings and maintains documentation. Participates in community education projects

  • Ensures that all necessary supplies and equipment are available

  • Uses statistical data to determine quality and quantity of services

  • Participates in personal professional growth and development

  • Ensures Agency personnel have current clinical information and current practices

  • Maintains current knowledge of local trends and issue

  • Must be able to perform nursing functions in times of staff shortage

  • Available to share on-call with nursing personnel as applicable

  • Ensures that documentation is completed in accordance with Agency policy

  • Assists with staffing visit assignments and on-call

  • Other duties as assigned


Required Skills/Abilities/Knowledge:

The Director of Patient Care Services shall qualify for the position by fulfilling the requirements under one of the following categories, unless the individual has been previously approved for such employment by a program flexibility issued for the individual's current position at the home health agency prior to April 1, 1995:

  • The Director of Patient Care Services shall have sufficient background knowledge and expertise in clinical decision-making for the patient population of the home health agency to meet the needs of his or her patients, and to contribute to Quality Management review and evaluation

  • The Director of Patient Care Services shall devote a sufficient number of hours to assure the quality and adequacy of services provided and supervision of staff


Education/Experience/Licenses/Certifications:

  • A registered nurse with a baccalaureate or higher degree in nursing or other health related field with three years of experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity; or a registered nurse with four years experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity
