The Clinical Supervisor (CS) is a qualified registered nurse or therapist (PT, OT, SLP) with the ability to oversee and coordinate provision of clinical activities for a designated group of patients and staff. In this role, you will report to the Director of Nursing (DON)/Director of Patient Care Services (DCPS).
Develops, supervises, and evaluates a group of qualified clinicians and caregivers to deliver clinical excellence in a patient-centered manner
Assists with the analysis and evaluation of adherence to clinical standards and Best Practices
Achieves goals as assigned regarding staffing, productivity, outcomes (as applicable by division: may include re-hospitalization, 5 Star and VBP processes), QAPI and care coordination
Responsible for care coordination in a case conference setting for high volume, high risk and problem prone patients
Participates in activities relative to provision of professional services to an assigned group of patients
Understands/adheres/educates on the QAPI and Compliance Plans
Assists DON/Administrator/DCPS to ensure that all staff providing care maintains professional standards of community nursing practice
Maintains and adheres to agency procedure and patient care policy standards
Assists DON/Administrator/DCPS with establishing policies and procedures in compliance with state health statutes and administrative rules to include CDC guidelines, OSHA guidelines for employee safety, standard precautions, and infection control procedures
Coordinates patient care services for an assigned group of patients to include workflow processes
Performs initial referral/intake clinical review which will result in a timely acceptance or denial of referrals
Provides case management as described in state regulations
Assists DON/DCPS with development/ implementation and keeps records of criteria for admission to service, case assignments and case management
Utilizes data dashboards as applicable to role
Informs appropriate staff about home health statutes and administrative rules
Assists DON/DCPS with installation of improved work methods and procedures to ensure achievement of objectives of the Agency
Ensures that all competency factors are assessed and corrected if applicable on a group of clinical staff members as assigned
Responsible for conducting initial clinical field staff interviews as directed by the DON/DCPS
Viewed as an expert clinical resource to a group of clinicians; this expertise may require CS to perform one on one instruction if applicable
Ensures oversight clinically of: making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized care plan(s)
Ensures that the following information/instruction is given in writing to the patient/caregiver/representative per payer requirements: visit schedule with frequency of visits, complete Medication Profile, any treatments to be administered by agency staff (POC), any other pertinent instruction related to the patient’s care needs, and name and contact information of the agency’s Administrator and DON/DCPS
Reports to the DON/DCPS regarding changes that might affect the clinical operations of the agency
Performs coordination as assigned during emergency situations and understands the Emergency Plan
Maintains confidentiality of agency, employee, and patient matters
Performs clinical visits if necessary and applicable; maybe necessary to cover for case manager of case as applicable
Assists with orientation of new staff prior to patient contact
Ensures participates in continuing education for staff to include orientation and in-services
Provides final clinical proofing of all documentation for a group of clinicians, if applicable
Assists/performs staff performance appraisals as assigned by the DON/DCPS
Assists with billing process as applicable from clinical perspective
Attains annual goals as established by the DON/Administrator/DCPS in the annual performance evaluation
Submits documentation within required timeframes
Performs other duties as assigned by the DON/DCPS
